Intensive In-Home Services are for children and adolescents between the ages of 4-21 years of age. These services are time specific and provides the individual with interventions to aid in decreasing the risk of being removed from the home setting. Services may also be rendered to individuals who are transitioning back into the home setting from a higher level of care, such as, a residential treatment facility, group home or juvenile detention center. The services we offer are designed to specifically improve the family unit and the interpersonal relations between the family members and the individual within the home. Services may be rendered in the home and community setting. Our goal is to provide the individual and family with the necessary tools and resources to improve the well-being of all involved. Services are free to individuals who receive Virginia Medicaid.


Criteria to be eligible for services:

Have difficulty in establishing or maintaining normal interpersonal relationships to such a degree that there is a risk of hospitalization or out-of-home placement because of conflicts with family or community.

Exhibit such inappropriate behavior that repeated interventions by the mental health, social services, or judicial system are necessary.

Exhibit difficulty in cognitive ability such that they are unable to recognize personal danger or recognize significantly inappropriate social behavior. For example, is at risk for acting out in such a fashion that will cause serious harm to themselves or others.

What to expect while receiving services:

Monthly Family Counseling with a Licensed or Licensed-Eligible Clinician

Specialized treatment interventions based on your specific needs

Meetings in your home by a Qualified Mental Health Professional 3-10 hours per week

24-hour on call Crisis Response and Intervention

Goals for treatment – as constructed by the family

Case Management – meeting with school, social services, etc

Conflict Resolution between family members
-
Community Involvement
-
Comprehensive Discharge Planning, to include linking with Outpatient Therapy, Medication Management, Mentoring, etc.
